Attendance Allowance (Special Services) in Israel (2026): who qualifies and how much

In short

  • A monthly Bituach Leumi benefit (קצבת שירותים מיוחדים) for people aged 18 to retirement age who need a lot of help with dressing, eating, bathing, moving around the house and hygiene, or constant supervision.
  • Four levels: ₪1,943, ₪4,501, ₪7,181 or ₪9,126 a month (2026).
  • It can be received together with the general disability allowance.
  • You can get up to 6 months back from the date of the claim.

Who qualifies

  • Israeli residents aged 18 up to retirement age. If you qualify on the day you reach retirement age, you keep receiving it. You can file for the first time up to six months after retirement age.
  • General disability recipients: a medical disability of at least 60%, or 40% with one impairment of at least 25%. Others: at least 75% medical disability.
  • Earnings up to ₪68,845 gross a month (5 times the average wage, 2026).
  • Not living in a medical, nursing or rehabilitation institution. During a hospital stay the allowance continues for 90 days.
  • Not receiving the special work-injury allowance or any other statutory payment for personal care.

Recipients with a child under 3 get an extra ₪1,215 per child, for up to two children. There are also additions for people on ventilators and for those employing two caregivers.

How the level is set

A Bituach Leumi doctor reviews your documents and, if needed, calls you to a medical committee for a dependency test. It checks how much help you need with daily activities and household tasks: preparing food, taking medication, shopping and errands. Only limits caused by your medical condition count.

How to apply

  1. Fill in the attendance allowance claim online, on paper or with a representative on *3928 who completes it for you.
  2. Attach medical documents listing all chronic conditions, treatments and medication. If you changed jobs in the last year, attach six months of pay slips.
  3. A Bituach Leumi doctor decides on the documents or calls you to a committee. Severe cases go through a fast track.

If you disagree

  • Appeal to the attendance allowance appeals committee within 90 days of the decision.
  • If you were refused for lack of medical disability percentage, appeal to the general disability medical appeals committee within 30 days.
  • Refusals for legal reasons, such as high income, living in an institution or asking for more than 6 months back, go to the Labour Court within 12 months.

Paid help

If you use a rights company or a lawyer, the law caps the fee as for general disability. A company may charge up to 11.25% of the allowance for the first 60 months plus a file-opening fee of up to ₪961. A lawyer who represents you at the committee may charge up to 13.75% plus the file-opening fee. VAT is added to all amounts.

Frequently asked questions

Can I get the attendance allowance without a disability allowance?

Yes, if you have at least 75% medical disability. Disability allowance recipients need 60%, or 40% with one impairment of 25%.

What happens at retirement age?

If you qualify on the day you reach retirement age, you keep receiving it. If you did not receive it by then, check the long-term care benefit instead.

Can I get it together with the mobility allowance?

Only in some cases: a 112% level or higher, 100% mobility limitation, or a need for a wheelchair as set by a Ministry of Health committee.

Are chemotherapy patients entitled to special services?

Yes. If you take a drug on Bituach Leumi's list, you automatically get the special services allowance at 50% for six months, based on medical documents only.
